Track Up to Four Debts Per Page
Each of the four sections gives you space to record the essential information associated with one debt or collection account.
For each account, you can record:
- Original Creditor
- Collection Company, if applicable
- Total Amount Owed
- Credit Reporting Agencies
- Payment Address
- Payment Arrangements or Settlement Total
- Phone Number
- Paid in Full checkbox
Keep Creditor and Collection Information Together
When a debt involves both an original creditor and a collection company, it can be useful to keep both names on the same record.
This sheet gives you a dedicated place for each, along with contact and payment information, so the details for that account stay together as you work through your financial records.
Record Credit Bureau Reporting
Each debt section includes checkboxes for Experian, TransUnion, and Equifax, allowing you to note which credit reporting agencies are associated with the account based on the information available to you.
This provides a convenient reference alongside the creditor and collection information without requiring a separate tracking page.
Track Payment Arrangements and Settlement Amounts
Use the Payment Arrangements or Settlement Total field to record the arrangement, amount, or other payment information you want to keep with that debt.
Once an account has been resolved according to your records, use the Paid in Full checkbox for a quick visual reference.
Print Additional Pages as Needed
