Medical Expenses with Auto-Calculating Totals - Printable

An editable PDF printable Medical Expenses with Auto-Calculating Totals page for keeping track of family or individual expenses for medical, prescription, vision, and dental all on one page. This editable page has 30 rows columns for date, cash, card or check number, short description of expense, and with amounts for medical, Rx (prescription), Vision, and Dental. The medical categories auto-calculate a total at the bottom of the page. There is also a Page Total at the top that sums every amount together as a whole. You can personalize this medical expenses record with the year at the top along with your name or family name and address if you want.

This page is offset slightly to the right in case you want to include it in a binder once printed.

MEDICAL EXPENSES PAGE INCLUDES:

• Individual or Family information (name and address). Families can also save this file separately and use for each family member if you need this information separated.
• Dates column.
• "Cash", "Card", or Check # column.
• Description of medical expense column.
• Columns for the amounts for all 4 categories: Medical, Rx (prescription), Vision, and Dental.
  
• Column totals for each category at the bottom (auto-calculates).
• "Page Total" at the top, which displays the sums of all amounts in the rows, and each category (auto-calculates).

⚠️ NOTE:
• When a category and/or row is not in use, use 0.00 in that amount column to help the Page Total calculate.
• The total is specified as a Page Total in case you need to utilize more than one page, more than 30 medical expense entries.
• You do not have to use all 30 rows (just keep 0.00 in the rows you do not use).
• You may wish to save a blank, 0.00 amounts, medical expense file for use the next year or for additional pages, to avoid having to delete all the fields when you start a new page/file.
• This is an editable PDF file, not a computer program. It is not interactive with any accounting software or any other program. Adobe Acrobat Reader is most recommended for all editing.
